How Do I Refer a Patient?

Mission Healthcare is a family of home health, transitional, and hospice care providers. We are committed to clinical excellence, compassionate care, and extraordinary service when life matters most. An earlier referral can lead to more care and support. Patients can be referred to hospice by a physician, family member or loved one, or can choose to refer themselves. However, a patient’s physician will need to provide a medical referral before services begin. Patients are admitted 7 days a week, 24 hours a day. All patient information remains confidential in accordance with HIPAA.

Next Steps

 

  1. Our admissions team will follow-up with the patient and/or family within 24 hours to understand the best level of care for your needs.
  2. If the person is eligible, our admissions team will assist in obtaining physician’s orders and necessary paperwork to begin care.
  3. Once admitted, our clinically-trained team works closely with the patient’s physician to coordinate care and ensure the patient’s goals are met.
